ChristianaCare is recruiting for an experienced Nurse Practitioner or Physician Assistant to join our Wilmington Adult Medicine Practice located in the newly redesigned and renovated The Rocco A. Abessinio Family Wilmington Health Center in Wilmington Hospital. The center provides primary and specialty care to all ages. One of the larger practices within the medical group, Wilmington Adult Medicine has 10 physicians, 3 nurse practitioners, behavioral health consultants and care managers working in collaboration as well as medical assistants and front office staff. This is a full-time position with the office having evening and Saturday appointments available. Candidate must be able to provide evening and Saturday availability in rotation with other staff.
PRINCIPAL DUTIES AND RESPONSIBILITIES
Assesses and documents patient and family response to intervention
Demonstrates ability to carry out critical skill as outlined in procedures and specific standards
Provides supervision and direction to other team members. Delegates task according to capabilities and scope of responsibilities.
Ability to independently review patient problems including, but not limited to medication refills, determining the need of follow up care and referring appropriately to the physician according to policy.
Performs all prescribed laboratory test, sends to appropriate laboratory, receives and interprets laboratory values, and notifies physician of results
Providing patient education and counseling to patients and their families
Participate in chronic disease management of patients
Demonstrates skills and knowledge necessary to provide care appropriate to (neonatal, pediatric, adolescent, adult and geriatric patients), including knowledge of growth and development, that ability to obtain and interpret information to identify patient needs, and provide the care needed.
Provide continuity and comprehensive primary care to a large population of patients across the lifespan in a resident physician family practice
Collaborate with resident physicians and co-manage patient care within the teamlet model
Mentor and precept medical, physician assistant, and advanced practice registered nursing students
Participate in evaluation and development of resident physicians as a member of clinical faculty
Participate in practice wide and team based performance improvement projects
EDUCATION AND EXPERIENCE REQUIREMENTS
Master's degree as an Advance Practice Registered Nurse -NP or graduate from an ARC-PA accredited physician assistant program and have NCCPA certification
Licensure or license eligible as a physician assistant or Advanced Practice Registered Nurse - FNP or AGNP as set forth by the state of Delaware
Prescriptive authority required in Delaware
Certification in BLS
Experience in Family or Internal Medicine as APRN-FNP, APRN-AGNP, or PA
